Q1. The system is apparently quite large in our terms of reference, being 'this end of the galaxy'. Hypothetically, if one were transported to a planet outside this galaxy for instance, then there would be a very good opportunity to exit without any post body death experiences similar to what happens here on Earth. However, one would still require to be aware and mentally focused and have some plan of action. Roaming aimlessly among the stars around this end of the galaxy, one could then be recaptured by tripping the system.
Q2. Very good question. There is indication that the system tags our personal ID marker on our first dump or trap within it as no two souls vibration is the same and it retains that record and everything about us from that very first time here, aka a timeline database built up after each death. The indication is the death construct then pulls all the information needed from 'our file' to present to us as images and emotions, etc, to gain our trust and then rinse and repeat each death. It is quite the ingenious creation when one ponders it, if it is true. It is suggested that this insidious system knows us better than we know ourselves, which would be understandable as we have no memories that are easily accessed to verify or deny the information presented at death as being real and most of the system population would not be remotely interested in researching this anyway and will just go along with whatever beliefs they have, eg: for many a religious savior figure is what is expected at death and therefore those are easily deceived and recycled. Not many souls know who and what they are, who they were and where from before Earth and how they arrived here and what has happened since that time. The memory wipe is similar to formatting a drive, the files are hidden, very few can be retrieved. It seems almost impossible to access a personal timeline to make sense of anything. If the system has been so successful since inception and deployment with only a very limited amount of souls leaving it behind, it will most likely continue to do so until such times when the tech is completely disabled.
OR... the whole thing is quite the elaborate psyop to trap those searching for answers, just to put things into perspective.
